ACTION PLANNING

At the Jackson Collaborative Network, we're committed to driving sustainable, long-term change by addressing the root causes of systemic issues. We recognize that many current systems were not designed to meet the diverse needs of all people in our community - often resulting in inequitable outcomes based on race, geography, and socio-economic status.

​To change outcomes, we must change the systems.

The Network provides tools, consultation, and support to help Network partners engage in, and lead systems change that helps them design and implement root cause level strategies within their organizations and within the community that help tackle these complex issues and barriers.  We call this process Action Planning - also commonly referred to as strategic planning.

What is Action Planning?

Action Planning is more than just setting goals. It’s about aligning your organization’s efforts with broader community priorities outlined in our Shared Measurement System Framework. This alignment allows us to:

  • Track and measure progress at a population level

  • Foster transparency and accountability among partners

  • Coordinate efforts for greater collective impact
