Partner Strategies

The strategies listed below represent specific actions being taken by organizations, agencies, and groups in the Jackson County community to root out barriers that are holding inequitable outcomes in place.  Click here to view the full action plan.

Shared Measurement Connections

The Shared Measurement System sets a common direction for change in Jackson County. It helps guide strategies and keeps efforts aligned. Partner strategies are organized by key aims and connected to the goals and root causes they support. You’ll see those connections outlined below.

Strategy or outcomes are aligned with Rx Kids strategic goals and priorities

Strategy or outcomes are aligned with Drive Jackson strategic goals and priorities 

Interviewing/collecting qualitative data from recipients of SUD services and providers of services.

Issue/Need:  

Making sure Jackson substance use disorder (SUD) programs and services are reaching intended audiences and populations, finding gaps and underserved groups/demographics. Using County data related to overdoses, hospital/ED visits, demographic data

SMS Goal:  Decrease years of potential life lost.

Services and policies are not equitable and do not benefit the most vulnerable populations

Primary Root Cause

Organizations and providers do not effectively communicate with community members

Secondary Root Cause

Collaborative Care Management for lower acuity patients to receive psychotherapy and medication management through their primary care offices.

Issue/Need:  

Access to mental health treatment and support. Looking at % of patients with identified mental health/behavioral health needs that can be seen within 14 days helped identify this need.

SMS Goal:  Increase % reporting access to quality health care.

Community members do not have the skills/assets and agency to get their needs met
